  • Select Option 1 and enter $205.00 on the Annual ISM/Affiliate Dues line.  New Membership total  is $225.00.
  • When finished, email your application to our Membership Committee Chair -   jaaffa@pplweb.com  and use the New Membership link below to submit payment via PayPal.
